The following letter is in response to "Quantum Leap: Transitioning From Pediatric to Adult Care" in the May 2015 issue of The O&P EDGE.

I am writing about a recent article Miki Fairley authored for The O&P EDGE. It was spot on! I am a nurse life care planner and, as such, I am charged with creating a lifelong care plan for individuals after catastrophic injuries or chronic illnesses. I always see the results of the fragmented, on-your-own-care that occurs after the kids age out of the pediatric hospital care system. I was impressed with the article and also the information from Texas Children's Hospital.

Our association, the American Association of Nurse Life Care Planners (AANLCP), is holding its conference in San Antonio in February 2016. Our theme is sensory disorders through the ages, and I think the premise of this article will be a great topic for us.
