Affidavit/Form for lost prosthesis

Hi! We have a patient who has lost his prosthesis in a creek and will be requiring a replacement foot. He just received a new foot about a month ago, and we were wondering if anyone has an affidavit form or something similar that we can put in the chart to justify a replacement foot/prosthesis to insurance?
