We have had really good luck with Arch Fitters. They accept molds, foam impressions, direct scans and scans of foam impressions. Reasonably priced for accommodative and functional and willing to correct problems. Hopefully this helps!

If you are looking at doing more functional type you could look into the PHITS Insole 3D printing system.

We are based in Australia but accept Stl files and have a cloud based foot orthotic modification software.
I am not sure what turn around time you are looking for?
Also with our exchange rate being 30% weaker than you we might work out being competitive even with freight.

for many many years we did hundreds of inserts a month. The key was to dedicate at least one person full-time to them. Depending on your volume the hassle with shipping and receiving impressions may be a problematic.
If you can routinely receive pairs of inserts for approximately $75 a pair you will be spending money every 30 days while probably taking 2 to 3 or more times that to receive money. Depending on your reimbursement sources it may be a losing proposition. If you only do a dozen or so a month then find two or three sources try each of them and evaluate them. But if you’re doing dozens of month you would have to do a cost-benefit to sending them out versus hiring someone even part time.

We work with Rick from Marathon Orthotics in Minnesota. He’s very accommodating and I believe they can receive scans. We shipped foam boxes back and forth with them and we could schedule a 13 business day turn around (which gave a little cushion for shipping) from send out to fitting to patient. They have foam that is also much cheaper than the competition for us which was nice as well. We’ve used him for active duty functional military foot orthoses as well as typical diabetic orthoses.

New Step Orthotic Lab. They have many types of inserts, including eva, and can do other materials if requested. Turn around time standard is 2 weeks, but you can pay for faster. Very good prices and good quality

Check out Summit Labs in Allegan, Michigan. I think they can provide you with exactly what you’re looking for.

I regularly use Comfortfit Lab and they do a great job. I have used them for
the past few years about 10 pairs/week.

I’ve been using footlogic for over a decade. Everytime I try someone else, I end up going back to them. They are a small company, and easy to talk to. Don’t know if they can work with scans. http://www.footlogic-inc.com/
