Patellectomy in TT Amputee with patella infera

Michael Arnette

Hello List,

I have have a patient who is considering the possibility of patellectomy
due to patella infera. He is soon to have an elective amputation below the
knee after years of fighting pain and infection in his ankle. His patella
cause no pain.
The patients patella has fractured into three pieces and is properly placed
at extension but upon full flexion it subluxates to the lateral side
reaching 1 cm proximal and just medial to the fibular head. The patient has
full ROM without pain.
It is not painful for the patient to flex the knee and when the patella is
manipulated and resisted in its displacement there is no pain during
flexion.

Would you recommend having the patella removed at the same time as his TT
amputation? I think I could provide a socket that would allow for movement
of the patella.

What has been your experience with patellectomy in TT amputees.
