Codes Denials

George Fagan

I have recently had 4 codes rejected by CMS: L8430 & L8440 and L5984 & L5986. Has anybody else had these codes rejected? They are questioning his K3 modifier. First time I’ve had stump socks denied. The other codes are the ‘axial rotation’ code and the ‘positional rotator’ code. Any replies would be appreciated.
