knee brace for 350 lb. patient

Hi List,
 
I have a medicare patient who is 6 months status post revision right total knee replacement.  He is 350 lbs.  Occasionally his knee goes out due to instability.  The doctor has ordered a custom knee brace, but I doubt Medicare will cover it.  Do you have any off the shelf suggestions?  His weight is obviously a factor.
