Alaska Workers Compensation Schedule

Hi All,
 
I have a workers compensation patient that was injured in Alaska, resulting in an amputation a long time ago.  I have searched online and it appears that you have to purchase the workers comp fee schedule from INGENIX, and that a new version will be coming out in the next month, also for purchase.
 
Do you have an electronic copy of the fee schedule you can email me?  Or: 
 
Can someone tell me the allowable fees for the following codes:  L5828, L5845, L5848, L5930?
 
Also, how do they treat unlisted Codes, L5999?
