I’m hoping someone can direct me to any Medicare policies regarding SNF
Consolidated Billing Exclusions and how an unspecified procedure code
(L5999, L7499, and L8499) is applied.
I’m looking for supportive documentation that indicates how unspecified
custom prosthetic componentry can be considered an exclusion when these
codes are not reflected on the list of exclusions.
