Dear colleagues, An insurance company referred a patient to me with an amputation of the right index finger at the dip joint. I sent for authorization using code L7499-upper extremity prosthesis not otherwise specified, I was told that they will not authorize this code as it is not part of the medicare fee schedule. Does anyone know a code that would be exceptable for this prosthesis. Thank you in advance for your suggestions sincerely Adam Canvisser cpo.
