Flail arm

Dear list:

Looking for some possible thoughts and or suggestions on a current case.

Patient presents with Rt. Flail arm secondary to nerve damage from multiple
radiation treatments for breast cancer. She has had a bi-lateral mastectomy
and extensive skin trauma and scarring to the right pectoral region.
Patient has developed lymphedema in the extremity that is putting a
tremendous amount of weight on the glenohumeral joint and shoulder girdle.
She is having a large amount of pain in the head and neck due to the weight
of the arm pulling on the trapezius and other muscles in the neck.

Her husband is concerned about long time use of pain medication and her
overall discomfort and well being. At this time they are exploring some
plastic surgery options for the damaged tissue itself, but nothing to help
with the pain and discomfort of the flail arm.

I am considering an elbow “type” orthosis/prosthesis design with an Monodos
joint that would allow for elbow positioning, which would be somehow
supported with an frame system on torso that connected to the brace above
the shoulder to reduce the overall weight of the arm and support it through
the body.

I would just like to hear if anyone has worked with something like this and
what the results were. Any ideas or suggestions would be greatly
