We are looking for a treatment plan template that incorporates normal process for new prosthetic intervention – to include the components for each stage with their necessity/justification and timelines; i.e., IPOP (and number of cast changes), preparatory prosthesis, definitive prosthesis and it’s components (ex; Dynamic response multi axial foot, vertical shock, torsion adapter, elevated vacuum, C-leg, etc…)
Any recommendations, CMS based source links, or sample templates that are acceptable by most insurances (CMS) would be greatly appreciated.
