Manufacturer of soft silicone ankle gauntlet

I have a patient that needs a soft, flexible, custom ankle gauntlet with silicone lined footbed and an accomodation for a partial calcanectomy.  Pt’s foot was mangled by a lawn mower.  Pt. had one made by Hanger in Florida which worked OK, but recently had a surgical revision.  Pt. is very sensative to pressure.  I would like a zipper closure. 
 
Another question regarding reimbursement.  This pt. is a dependent of an active service member.  How would you recommend billing this device to TriCare for adequate reimbursement and have any of you been paid for these.  I know these aren’t cheap to have made and would rather not lose money on the deal.
