This message was just forwarded to me by the Florida Department of Health
regarding license renewal. The renewal date is November 30th. Make sure
you don?t miss the deadline.
Instructions for renewal are in the message.

For the current Prosthetist-Orthotist renewal cycle that will end November
30, 2003, only 56.71% have renewed by November 3, 2003.
For the current Prosthetist renewal cycle that will end November 30, 2003,
only 45.95% have renewed by November 3, 2003.
For the current Orthotist renewal cycle that will end November 30, 2003,
only 44.62% have renewed by November 3, 2003.
For the current Orthotic Fitter renewal cycle that will end November 30,
2003, only 27.73% have renewed by November 3, 2003.
For the current Orthotic Fitter Assistant renewal cycle that will end
November 30, 2003, only 39.02% have renewed by November 3, 2003.
For the current Pedorthist renewal cycle that will end November 30, 2003,
only 27.54% have renewed by November 3, 2003.
The Department of Health believes that more licensee?s plan to renew, but
may not have received their renewal notice due to an incorrect address or
may be delaying renewing until later this month.
To ensure a timely renewal, licensee?s may renew using MQA On-line Services
by visiting HYPERLINK
“http://www.doh-mqaservices.com/”www.DOH-MQAServices.com. The system will
be available to these licensee?s until Midnight, November 30, 2003.
MQA On-line Services account ID and passwords can be found directly on the
renewal notice. If the licensee did not receive their renewal notice, the
Department can provide the licensee with their account ID and password over
the telephone or by email. To receive an account ID and password over the
telephone, the licensee should contact the Department at (850) 488-0595 and
press menu option 3. A member of the MQA Call Center can assist the
licensee with this information. To receive an account ID and password by
email, the licensee can email their request to HYPERLINK
