knee brace

i have a patient with a difficult knee problem. he has been wearing a acl brace for over a year now . he has had many types of knee problems. currently he has bil ,lat djd oa and no acl or pcl he is currently scheduled to have a toal knee reconstruction sometime this summer but he has just undergone major gi surgery and is constantly losing weight . this is the first time that i have seen this patient and the acl brace that was ordered for him had to be moderatly modified to allow for his leg vircumference and the angulation of his knee. x-ray shows that he is currently bone on bone i have casted hime for a custom fit brqce and have all his measurements and i have pictures from a digital camera . I woul kile to know if there are any suggestions for this type of patient or if any one has worked with a patient with these multiple instablilites and what do you do to help them function again my patient is ambulatory.
