Medicare 5 year Afo ABN

Afternoon,
Let me begin with Medicare is absolutely frustrating!
Patient was seen and evaluated for an AAFO. Two months ago, she received an Aircast ankle walker. She is leaving town in two and requested her orthosis be ready. Now for the issues
She is not eligible under Medicare‘s 5 yr, same and similar rules. She is requesting the provider of the Aircast ankle walker return the money to Medicare and she will pay. Since she wants to receive the custom AAFO in 2 weeks. At the time of delivery, she will not be eligible for Medicare submission or reimbursement. My concern- patient is a little slippery.
What should I anticipate?
ABN form- how detailed?
Any concerns?
