Proper Medicare Info from Doctors

I am interested in finding out how others are getting their prescribing
physicians to provide all of the required documentation needed for
Medicare reimbursement. Specifically with our prosthetic patients, we
are unable to get some docs to give us the info needed.So we developed a
form that has all of our prosthetic evaluation findings. Then we send
over a version with all of the Answers if you will on it and an attached
letter stating that we need them to add to or agree along with any other
medical history they may have. We also send a blank copy of this form
for them to transfer in their writing our findings prostheticly,along
with doctors added comments and medical history.

Can we send this form over and then just have doctor fill in the medial
history and sign it?

The paperwork burden for all involved seems to be unending.
Please give us some insight into how you are working with this
situation, I will post any responses with names omitted.
