Limb Volume Change: A Daily Reality

Residual limb volume variation is not an occasional issue, it is a constant clinical reality.

Throughout the day, patients experience volume changes driven by multifactorial causes. Over weeks and months, muscle atrophy, tissue remodeling, and healing processes further alter limb shape, making it extremely difficult to keep a perfect fitting socket throughout the day.

The Limits of Traditional Volume Management

Traditional volume-management strategies, primarily prosthetic socks, provide uniform bulk. They do not address localized voids and typically require removing the prosthesis to adjust. This can interrupt daily activities, reduce comfort, and negatively impact alignment and suspension.

A More Dynamic Approach to Fit

The Ethnocare Overlay is an air-based adjustable prosthetic device designed for both transtibial and transfemoral patients. It allows users to fine-tune socket fit in real time, without removing the prosthesis. Rather than adding uniform layers, the Overlay accommodates volume changes in a targeted way, helping improve suspension, reduce pistoning, and enhance overall comfort.

This capability is particularly important for new amputees. Early rehabilitation is a critical period for building confidence, stability, and consistent prosthetic use. Unmanaged volume fluctuation during this phase can lead to instability, skin irritation, and reduced wear time. Real-time adjustability supports a more consistent socket fit throughout this transitional period.

Reimbursement and Clinical Recognition

In the United States, the Overlay is now covered under HCPCS code L-5657!

During the coding process, CMS recognized the Ethnocare Overlay as the predicate product for this new category, acknowledging that Ethnocare’s products as a distinct solution separate from traditional volume-management approaches. This recognition supports broader reimbursement access across Medicare and many commercial insurers.

Clinical Evidence Supporting the Overlay

Clinical evidence supports the Overlay’s effectiveness. A peer-reviewed study published in Sensors (2024) demonstrated improvements in comfort, stability, and functional performance compared to prosthetic socks, with reduced pain and compensatory movement strategies.
