Hello All,
Can anyone point me to the exact page in the Medicare manual or LCD that states a Physical Therapist’s treatment notes must be co-signed by a physician to be accepted as part of the medical record to justify the need for a custom orthosis. The physician is annoyed that he would be required to sign another professional’s documentation and the therapist is insulted. Performant states this is a requirement and they would both like proof in writing.
Any assistance would be appreciated.
