BIlateral TFA

Greetings – I’d like to posit a query of experience my traveled and tested
mates may have encountered, and their successful means of accomplishing said
task:

I have encountered a patient, 73 yrs old, bilateral TFA of less than two
years. Both limb losses were spontaneous to pedestrian vs. MVA. Patient
was in a crosswalk. No prior prosthetic intervention. Current weight is
over 300 pounds and a pre-accident morphology. Independent in all regards
prior, self-reported. Retired.

Reports independence at home with a power wheelchair recently provided with
accommodations in-house for facile transfers and modest access to his
surrounds, including overhead suspension methods for UE assist where needed.
Having no means, nor access on-site in my facility to apply these overhead
measures for transfers and the allowance of safe mobility, it seems at
first glance that any attempts to initiate care would be from his chair,
already challenged for space in regards to opportuning body shifting,
self-elevation etc..

Anyone had this clinical experience with successful outcomes? Liabilities
without the proper means for mobile access in fitting pursuits despite the
best intentions? Alternate facilities that accept this clinical presence
even if not service connected?

I can’t discharge my pursuit to assist him without more Community knowledge
that might better inform me of some modicum of potential for him. Apologies
for the extended missive. I appreciate your
replies/impressions/recommendations.
