questions about a few KAFO L-codes…

William Lifford

Hello colleagues,

I recently had a discussion about KAFO coding with an orthotist (who I see
at a clinic, with whom I do not work) and he had very different ideas about
coding. Basically, the core of the issue is that I feel like his
assertions are too aggressive, but I am not an expert on orthotics coding
so I would love to get your opinions on these questions.

1. Does heat-shrink tubing count as “non-corrosive finishing (per bar)”?
My thought is no, that the code is really more for powder-coating and other
applied finishes, but I don’t really know for sure.

2. For a plastic AFO or KAFO, if you have the L2275 varus control (mods to
the plastic/trimlines, etc), can you also bill for the L2270 varus control
strap? Again, my thought is no, because the illustrated guide makes it
look like a T-strap used in conventional metal-and-leather KAFOs (for which
the 2275 code wouldn’t apply). Also, in a plastic AFO/KAFO, would the
L2270 apply only to a slotted strap, or would it also apply to a classic
“45-degree pull” instep/ankle strap?

Thanks so much for any advice or clarification you can give. I’ll post a
summary of responses if people wish.
