Fidelis coverage – NY

Hi,
Has anyone had coverage for the primary myo code L6935 with Fidelis? We
obtained authorization for all the codes associated with the prosthesis but
were paid on the add on codes not the primary code.
We have other patients that are in need of a myo and are asking them to
seek other insurance coverage.
