Has anyone created a form for a prescribing physician to complete to cover the required Medicare documentation for a prosthetic limb AND used this at an ALJ level as the dr’s documentation? If so, how did the judge feel about the physician completing the form as the documentation?
I know Medicare does not care for templates. However, I feel a template would cover more required info than relying on the dr’s to cover everything Medicare is looking for.
I am thinking about creating a simple form for our doctors. The form would not simply be a template w check boxes but would also include blanks for the dr to complete detailed info.
