Hi everyone we are in Region C. We recently billed Medicare for a humeral fracture brace, using L3980, L3995 with LT and the KX modifer. 812.00 being the dx. The claim is coming back as needing additional information for these two codes. I am at a loss here. Are there additional modifers we should be using? As always, thank you kindly for your help.
