Getting to the Truth

As a clinician, your daily work is built on trust. Every patient brings a unique story, and it’s your responsibility to listen, interpret, and make decisions that impact their lives. The challenge of discerning truth—separating fact from perception or misunderstanding—is central to a successful patient care plan. Discernment is as vital in the clinic as it is in navigating the broader world of information. Listening for Truth in Patient Stories The art of discerning truth begins with how we listen. Active listening is more than hearing words; it’s about understanding context, emotion, and intent. When a patient describes their symptoms, we must be present and attentive, asking open-ended questions like, “Can you tell me more about what you’re experiencing?” This approach encourages patients to share detail and nuance, which helps identify inconsistencies or gaps that may need further exploration. Triangulation is another cornerstone of clinical discernment. Rather than relying solely on a patient’s account, we gather information from multiple sources: family or caregiver observations, objective measures like ROM and physical measurements, and previous medical records. By comparing these sources, we can spot discrepancies or patterns that either support or challenge the patient’s narrative. For example, a patient might report severe pain, but objective findings and observed behavior may not align. This doesn’t mean the patient is untruthful, but it signals the need for deeper inquiry and a broader perspective. Bias—both conscious and unconscious—can cloud our judgment. Clinicians must be vigilant about biases such as confirmation bias (favoring information that fits our preconceptions), anchoring bias (relying too heavily on the first piece of information received), and attribution bias (judging credibility based on appearance or background). Recognizing these tendencies helps us remain open to the full complexity of each patient’s story and avoid premature conclusions. Context is equally important. The truth of a patient’s story often lies in its broader context—social, cultural, and psychological factors that shape how symptoms are experienced and described. Asking about social determinants of health, such as housing, employment, or support systems, can reveal underlying issues that influence the narrative and help us discern what’s really going on. Parallels with Discerning Truth in the Information Age The skills we use to discern truth in patient care are just as relevant when evaluating information from news, research, or social media. In both settings, critical consumption is key. We must approach information with a critical ear, asking: What is the source? What is the context? Are there emotional cues or persuasive tactics at play? Rather than accepting headlines or soundbites at face value, we look for depth and nuance, seeking to understand the full story. Cross-referencing is essential. Just as we triangulate patient stories, we verify information by checking multiple reputable sources before accepting a claim. Consistency across scientific literature, expert opinion, and primary data builds confidence in what we know. We should be wary of information that stands alone or contradicts established knowledge without strong evidence. Bias awareness extends beyond the clinic. We must recognize our own filters when consuming information: Are we seeking out information that confirms our beliefs? Do we give more weight to sources that align with our worldview? Are we dismissing information because it feels uncomfortable or unfamiliar? Challenging our assumptions makes us more objective and discerning. Contextual thinking allows us to see beyond isolated facts and grasp the broader truth. We consider who is providing the information and their motivation, the historical or scientific background, and whether there are missing pieces that could change the interpretation. Practical Tips for Clinicians and Healthcare Leaders While most of our discernment happens in conversation and reflection, a few critical habits are worth highlighting: Pause before reacting. Take time to process information before making judgments or decisions. Ask clarifying questions. Whether with a patient or a piece of news, dig deeper to understand. Document your reasoning. In clinical notes or personal reflection, record how you arrived at your conclusions. Fostering a culture of inquiry—encouraging colleagues and staff to question assumptions and share diverse perspectives—strengthens our collective ability to discern truth. Conclusion: Truth as a Collaborative Process Discerning truth, whether in patient care or the wider world, is not a solitary pursuit. It thrives on curiosity, humility, and open-mindedness. As clinicians, we are uniquely positioned to model these skills for our patients and communities. By honing our ability to listen, triangulate, recognize bias, and contextualize, we not only improve patient outcomes but also become wiser consumers and stewards of information. In an era where truth can feel elusive, these are the tools that keep us grounded, compassionate, and effective in all that we do. Let’s continue to champion discernment—not just as a clinical skill, but as a way of engaging with the world, one story, one fact, and one patient at a time.
