Capital Prosthetic & Orthotic Center, with four offices in central Ohio, had over 1,100 patient records stolen from a storage facility in Zanesville, Ohio, on December 12, 2016. The company told the Times Recorder that the files, stored in boxes, were for patients seen between 2008-12, and the stolen information may have included personal details, such as names, addresses, social security numbers, insurance information, and treatment information. The Times Recorder also reported that all affected patients were notified, as was the U.S. Department of Health and Human Services’ Office for Civil Rights, in accordance with the Health Insurance Portability and Accountability Act of 1996.
