Rx Pads Given to Doctors – what=?utf-8?Q?=E2=80=99s_?=appropriate?

Hello everyone,

What are the rules regarding custom Prescription pads given to doctors?

I notice a Juzo Rx in front of me, for example has their name and website address on the front of their form.

What are we allowed to do?

Address and contact info on front and/or back of Rx?

Anyone have an Rx they can show?

Or a place where the rules are presented very clearly?
