I have a claim that was billed to UHC for a replacement socket. They have
paid all codes aside from the base code L5700.
I have received a request for additional information and it is not your
typical request that we have ever come across. Please see below and advise
if you have any suggestions. I am not really sure what to send considering
the supplies for a replacement socket are purchased in bulk.
