United Repair modifiers

Hello
We are getting repairs denied by United Healthcare, Medicare and Medicaid
type replacement plans, stating that we are using the wrong modifiers.
We’ve used LT, RT, KX, and nothing and every combination of them, still
nothing. Does anyone know which modifiers we are supposed to be using for
repairs? We are in Washington State.
