These are the items that require a face to face six months prior to a written order; all DME. 
Although it is highly recommended that the patient see a physician prior to a order; to obtain proper evaluations and justification in the medical record for items being ordered. All medical records should be contemporaneous within 12 months of the date of the order.
When one is working with an amputee there is just reason for the amputation, the residual limb has many changes over a short period of time. Many diseases require continued treatment (treatment and follow up sooner than one year), remembering here that Medicare wants a clinical course of action for the patient (the human body is not just one thing it is a system or many parts) what affects one will possibly affect others (the limb or the use of the limb).The DME list of Specified Covered Items is as follows.  The original list was at 77 FR 44798.  This original list contains some codes that have been deleted or that are not valid for Medicare (*) in the interim while some other codes have had narrative changes (**).  Updates to the list will be made as CMS releases revisions. Refer to the Pricing, Data Analysis and Coding Contractor web site for information on coding at www.dmepdac.com.
