Working as a 1099 employee

What are the rules regarding working as a 1099 CPO? I know that Diabetic
Shoes must be fit by a W-2 employee. What are the rules for providing
custom prosthetic and orthotic devices? Additionally, what is the
implication for an employee in this situation allowing the facility they are
working for to operate under their certificate?
