Medicare and carbon fiber afo

Correct me if I’m wrong Medicare guidelines are orthotics services can be provided with 5 yr rule same or similar.

If a Medicare patient Is provided with a pre fab afo and fails after two yrs

Warranty is no longer valid
What is the correct way to provide service and get reimbursed from
Medicare

Please provide any suggestions
