Distal end pain on TT patient

List,
I have a bilateral 71 yo female TT patient, amputee for 10 years or longer. Has atrophied to very conical pointy distal ends with no tissue between skin and bone. Right limb looks similar to left, but not much problem. Left has been problemsome for years now, maybe 3, and cannot get her comfortable for longer than a few weeks at a time. She is in cushion liner from alpha 9mm and ossur internal socket padding. total contact, but less pressure ditally than proximally. Tried everthing, surgeon says lack of distal tissue it the reason. Any suggestion to alleviate pain would be appreciative. Stump shows signs of excess pressure but playdough and lipstick testing shows that it is not so. I don’t think it is hammocking.
