Unique Locking Mechanism

Dear List:

Thanks to many of you who responded and I apologize for not ruling out the most obvious two answers: the lanyard system and the Coyote Lock. The patient rejected the lanyard as too much hassle and the Coyote as too bulky.
Some of the more interesting suggestions are below. At least one manufacturer offered a prototype they have in process and one practitioner offered a design they developed in his own facility. The responses follow:
