We fit a thumb-wrist brace to the Medicaid patient on Oct 2019 with the billing code L3807. This code does not need pre authorization. Our claim has been denied first, and after we request for appeal, now they want us to provide them with our invoice!
– Is this something that insurance companies can ask?
– What if we purchase the item long time ago and can’t find the invoice? What if the manufacturer provide an item to us for free?
Any suggestion would be really appreciated.
