DM shoes CASH pay

For those of you providing diabetic shoes as a CASH service, what kind of things do you need to be aware of? Do you bill Medicare other services and not for Shoes? Is there a separate tax ID # that provides the shoes that is not contracted with Medicare? Just looking for an alternate way of still providing this service and not going crazy from doing all the paperwork.
