NYS Workman’s Compensation adopted physician request authorization form
C4 Auth. I have been unable to obtain a straight answer.
Has anyone been able to obtain C4 Auth number?
How are we able to negotiate our fees, if we are excluded from the
authorization process with workman’s Comp.?
Medicare Questions:
If at all possible, Medicare has become less helpful!
We are recently encountered two claim denials. Claims were submitted to
Medicare for L1970, L2820, L2220 and L2275. Medicare denied claims
because patient received 6-8 months prior L1902 or L1906 claiming same
or similar. This is absolute nonsense! Both these patient’s presented
with significant issues. They have both met the basic and custom
criteria under Medicare LCD L33686. Â
Any recommendations?
