Extra liners for collars and CTO’s

Hi List,
 
For those of you that routinely fit Miami J’s, Aspen’s and CTO’s do you automatically provide extra liners.  What reimbursement code do you use and do you actually get paid for them?  Medicare doesn’t pay for extral liners and it seems that some insurance companies are denying them.
