I have been referred a 62 year old male for a “Spondylolysthesis Brace” for a pars defect at L4-L5. Ordinarily the orthotic solution for this is reasonably straightforward, but this individual is 6 years post successful kidney transplant .The new kidney is positioned subcutaneously in the lower portion of the epigastric fossa. It is quite palpable from the inguinal fold proximally to roughly the level of the umbilicus. His nephrologist has actually prescribed that the patient wear suspenders rather than a belt because the nephrologist does not want any pressure or constriction applied to the lower abdomen. I am at a loss to think of a spinal orthosis that would apply sufficient force to stabilize the lumbar spine without exerting pressure in the epigastric fossa. I would appreciate any input from anyone who has successfully managed a similar case.
