Salary Survey 2003

If you are an owner, director or manager of an O & P facility or
department and would like to be part of an informal study, please
respond to the following questions.

This study was originally done in 1999, in which 14 facilities
participated. With positive feedback received from facilities that
submitted and then received results.

I will e-mail results only to individuals that have responded to this
survey, results will not be posted to the Listserv. Please type your
e-mail address with a space before and after it for a reply, ex.

Please send results to the E-mail address in example not to Listserv and
list your full name and title and e-mail address for replies.

Results will not list facility names, they will only be listed by state.

Results must be in by July 31, 2003 to be included in survey.
