I did not receive the survey in the first instance, but I have since seen a copy and am disappointed to learn that the authors did not recognize at the end of the document one of the most established philanthropies of the O&P community, the Orthotic and Prosthetic Assistance Fund, Inc.
The Fund will be launching its new web site very shortly at www.opfund.org
