Suggestions for Elevated Vacuum Cosmetic Appearance

Sagar Shetty

Hello List,

We have a below knee patient with a bony stump and lot of soft tissue in
the thigh. We made a custom liner (non-fabric-covered) for her from
Evolution Liners, they don’t make liners with fabric. We used the Otto Bock
Harmony E-pulse for elevated vacuum and the Otto Bock Harmony Sleeve (that
was the most firm sleeve I could think of). Patient is very happy with the
fit and function, but with this elevated vacuum, the soft tissues of the
thigh form a large roll just above the socket trimline and the patient does
not like the cosmetic apperance of it. If I pull and hold the sleeve up on
her thigh, the roll reduces, but as soon as I let go, the roll drops back
to where it was. The patient is not willing to use a waist suspension for
this. I have attached pictures of the fitting and her stump.

Does anybody have any suggestions/recommendations to tackle this issue? Any
response will be appreciated.

Thank you.
