Good Afternoon,

I am currently on a committee that is investigating the possibility
of tracking the in-house volume of orthotic/prosthetic work in a manner
similar to what is currently done in private practice.

If you have blank orthotic and prosthetic forms (electronic
preferred) that list L-codes along with the device description and
additions, please consider emailing them to me.

Preferably these would be separate files for Spinal orthoses,
transtibial, symes and partial feet, lower and upper extremity orthoses.
