Does anyone use a standard form for recording a patients medical and O&P
history that you like? We are trying to create some efficiencies and
consistencies within our offices. Some practioners spend too little time
obtaining necessary information then wonder why insurance companies request
additional information all the time. If we all used the same form or set of
questions I think this problem may decrease greatly.
