I forgot to mention that the full version of the online survey can be seen

The text version that was sent through this list did not include the
multiple choice text boxes. Some of the questions are multiple choice some
are open ended.
Just in case you are wondering, apart from my own business, I work part
time for Canada’s largest market researcher The Angus Reid Group. Many of
the questions I have asked in my survey are based on questions asked in
most Angus Reid telephone surveys.
